My local muffler shops can't seem to help, and a local pipe fabrication company wants to charge me about $200 to fabricate a collector.

It looks as though you could go in from the side of the manifold, where the two passages run together and bore a hole that splits the difference between them. The hole between the passages would be just big enough for the O2 sensor, then widen out for the threads. The tap size is M18 X 1.5 (large spark plug size) You would also need to spotface the surface of the manifold as a sealing surface.
Something like this:

The advantage of this is close proximity to the engine and a good solid spot to mount. If the hole around the sensor part were held to just larger than the sensor, it should not mess up flow very much. I may do this, just haven't found the tap.<!-- google_ad_section_end -->

On mine, the collector is about 8" down from the flange that connects to the exhaust manifold, and the muffler shop welded the twin pipes to the 2-1 colletor.
I remember him calling his supplier one day before he build mine and requested a 2-1 collector, and he didn't went much into details, so the collector must be kind of generic, I don't remember the diameter.
You may want to ask your local muffler shop, good luck.
