of course you probably will find little horrors under the paint..it's part of the history of an old car..

You really need to remove the door handle (which looks mis -shapen / bent or it could just be the angle of pic) and get under that patch and take off the paint from that area and assess from there, until you remove the paint you're just guessing.

It looks like there is also a small dent on the rear door edge

most times the damage is a depression around the handle that rather than pushing out prperly, somene has fliled. The edge of the handle looks like it's dug into the pant creating the point for water ingress underneath and into the filler.

The issue is getting the recess for the handle nice and square / straight and with the correct profile... it's far from a flat panel just there.

If the interior door panel is removed, you should be able to see that area. That should give you a better idea of the extent of the repair and allow you to see any rust on the inside.
